Advance Care Planning and Paperwork
Advanced directives are legal documents that designate who will make choices for you when you are unable and they provide guidance for your healthcare providers. Communicating your wishes and values while you can, before a crisis will protect you and your loved ones from unnecessary suffering. I recommend that all adults prepare these documents and revisit them with any major life or health event. Give your the loved ones the opportunity to honor your life and values by following your wishes.
I will assist you in choosing a health care agent who will best represent your wishes and your values.. We will employ a values-based process of exploring your preferences using real-world scenarios and recommendations from leaders in medicine and patient advocacy. Topics we will cover include life support, palliative care, dementia, organ donation, alternative treatments, DNR, VSED, and pain management. I provide you with resources and tools for having these essential conversations with your health care providers and loved ones.This takes a minimum of two sessions and can be done in-person or via phone and email.
Life Review and Memorial Projects
Memorial projects are meaningful and important at the end of life. These projects can be made with or for your loved ones. I will assist you in designing and completing a gift that will remain in the world after death. Options include : letters, ethical wills, celebration-of-life events; formal oral history interviews, digital archives, playlists, and memory books. Imagine these experiences and objects comforting and inspiring your loved ones and future generations.
End-Of-Life Coaching Package
I work with individuals and their care circle to design and implement a sustainable and flexible plan grounded in your values, resources, and evolving situation. I help you get your affairs in order so that you can spend your days focusing on what is most important for you. We will plan and prepare for what we expect is ahead while making space for the unknown and mysterious. I will guide you through the following tasks:
- complete/update advance directives.
- life review or legacy project
- disposition and funeral planning. I provide information about all options, including home funeral, natural burial and help you to navigate the funeral industry and the law.
- identify and coordinate additional support and resources
- plan follow-up processing with your support circle
Bedside Vigil
During the final stages of the dying experience I work in coordination with hospice/medical care in your area. I help you create a plan and I’m on-call to guide and support your circle of care. This is a challenging and sacred time. We all deserve the information and support that enables us to experience it without trauma.
After-Death Care and Ritual
I help loved ones secure the time they need with their recently deceased. This after-death care may include guidance in preparing the body, filing legal paperwork, and creating a space to gather and to say good-bye. I provide information to implement safe and legal procedures according to state law and local resources. The emphasis is on minimal, non-invasive, and environmentally friendly care of the body.
All services offered are non-medical.
I do not provide services that require a licensed funeral director but I can help you to select someone who will support your needs.
