Advance Care Planning and Paperwork
Advanced directives are legal documents that designate the person who will make choices for you when you are unable, and provide guidance for your healthcare providers. It is recommended that all adults prepare these documents and revisit them with any major life or health events. Advance directives may protect you and your loved ones from unnecessary suffering or help you to remain at home at the end of life. They may ensure that you are able to contribute to the advancement of medicine, or to saving another life when yours has ended. Illness, death, and dying are scary and overwhelming for most of us. This is a way for you to provide some comfort and solace to your loved ones, enabling them to honor your life and values by following your wishes.
I will assist you in choosing a health care agent who will best represent you and in communicating with them about your wishes. We will employ a values-based process of exploring your preferences using real-world scenarios and recommendations from leaders in medicine and patient advocacy. Topics we will cover include life support, palliative care, dementia, organ donation, alternative treatments, DNR, VSED, and pain management. I will provide you with resources and tools for having these essential conversations with your health care providers and loved ones.This takes a minimum of two sessions and can be done in-person or via phone and email.
Life Review and Memorial Projects
Memorial projects provide a meaningful and generative focus at times when there is a tendency to attend only to the challenging impact of aging and illness on the body. I will assist you in the enlivening process of designing and completing a gift that will remain in the world after you die. Memorial art projects can be made with or for your loved ones. Options include : letters, ethical wills, celebration-of-life events; formal oral history interviews, digital archives, playlists, and memory books. Imagine these experiences and objects comforting and inspiring your loved ones and future generations.
End-Of-Life Coaching Package for people facing terminal illness and their care circle
We will work together to design and implement a sustainable and flexible plan for you and your circle of care. grounded in your values, resources, and evolving situation. We will get your affairs in order and supports in place so that you can spend your days focusing on what is most important and enlivening for you. We will plan and prepare for what we expect is ahead while making space for the unknown and mysterious. I will guide you through the following tasks:
- complete/update advance directives.
- life review or legacy project
- disposition and funeral planning. I provide information about all options, including home funeral, natural burial and help you to navigate the funeral industry and the law.
- identify and coordinate additional support and resources
- plan follow-up processing with your support circle
During the final stages of the dying experience I provide non-medical support in coordination with hospice/medical care in your area. I will help you to create a plan and be on-call to guide and support your circle of care. This is a challenging and sacred time. We all deserve the information and support that enables us to experience it without trauma.
After Death Care and Ritual
I provide information and tools to enable loved ones to take the time they need with their recently deceased. This after-death care may include guidance in preparing the body, filing legal paperwork, and creating a space to gather and to say good-bye. I provide information to ensure safe and legal procedures according to state law and local resources: The goal is to facilitate maximum involvement of loved ones and their social network. The emphasis is on minimal, non-invasive, and environmentally friendly care of the body.
All services offered are non-medical.
I do not provide services that require a licensed funeral director.
