Advance Care Planning and Paperwork

Advanced Directives are legal documents that name the person you designate to make choices for you when you are unable and provide guidance for your healthcare providers and loved ones. It is recommended that all adults prepare these documents and revisit them with any major life or health events.

I will assist you in choosing a health care agent who will best represent you and in communicating with them about your wishes. We will employ a values-based process of exploring your preferences using real-world scenarios and recommendations from leaders in medicine and patient advocacy. Topics we will cover include palliative care, dementia, organ donation, life support, alternative treatments, life support and DNR, and pain management. I provide you with resources and tools for essential conversations with your health care providers and loved ones.

This typically takes a minimum of two sessions and can be done in-person or via phone and email.
